We have a marketing survey that asks whether the respondent has seen various print advertisements. The paper version shows a thumbnail of each ad, with a checkbox next to each for the respondent to indicate Yes or No.
I've approximated this behavior using a sequential set of pages:
1. Explain that we will show the user a picture, then ask Yes or No afterward.
2. Add a label field field with the image of the ad and a title.
3. Add a Yes/No question afterward - do you recall seeing (title).
4. Repeat for each of the four ads.
The survey would work better if we could add both label text and an image to the choices of a Choose One or Select Multiple question. It would be fine if you made limitations of a few fixed pixel sizes - say 75x75, 100x100, and 150x150.
Please consider adding this feature to the DoForms software.

We'd be fine with the option to add an image to a Choose One question. That would like like "Do you recall seeing this advertisement in print? (o) Yes (o) No" with the image following. That way, we could have the user cycle through a series of images and mark the ones they do remember seeing. -

Understood - I have a work-around ready for our 1/1 deployment.
I'm sure the project stakeholders will ask whether an improved user experience is forthcoming. I'll tell them I've shared the issue with you and that you are researching feasibility of adding it to DoForms.

Just one more thought, we always provide the option for customers to have their requests expedited via an engineering fee. Let me know via email (mark@doforms.com) if this is something your customer is interested in. Otherwise, we will get it done. Just not sure when.
